A handoff should tell the next person what changed, what is open, what is blocked, and where to inspect the work.

Where notes come from

Ask an assistant connected through MCP, such as Claude or ChatGPT, to draft handoff notes from your current initiatives, bugs, comments, and blockers. The notes are only as good as that work is current.

Agent context in notes

For agent-assisted work, preserve the original work item, the agent session, links to output, and any human review still needed.

Put context that should outlive the handoff in a document attached to the work.
