Use handoff notes to preserve current context when work moves between people, teams, or time away.

Capture the current state

A useful handoff note captures current status, in-progress work, planned work, blockers, active initiatives, key contacts, relevant documents, and links back to the source tasks or issues.

Where notes come from

We can generate handoff notes through MCP prompts using completed, planned, blocked, and initiative data. Notes are strongest when tasks and initiatives already have clear titles, descriptions, owners, taxonomy, comments, and blockers.

Agent context in notes

For agent-assisted work, preserve the original task, agent session, claim status, activity updates, external URLs, completion or failure state, and any human review needed before the work is considered done.
